Which personal information we collect from you

 A) During an appointment

Any relevant personal information collected at the initial appointment time by ourselves is a legal requirement. This information is collected and recorded hard copy on a patient record card. This process is one to one with the patient and Kelly Ashforth only. Personal information we collect includes name, address, email address, date of birth, diabetic status, any allergies, medications, current infections, and the name and address of your GP.

 B) When you register with us or place an order with us, we collect and store the following basic information:

  • Your name
  • Contact details e.g. Address, telephone number, email address
  • Order details
  • Username and password
  • Payment information

You may provide us with additional information when you communicate with us by telephone, email, post and by using this site for example if you store a list of favourites or complete a questionnaire.

 

How we use your Personal Information

We use this information to fulfil our agreement with you for example:

  • To diagnose any foot health conditions and ensure any treatment we give you is suitable based on medical conditions or medication taken for your safety  
  • To register you with our website and to administer our website services;
  • To process your order;
  • For assessment and analysis to enable us to develop and improve our services;

Cookies

This data would only ever be shared with a consultant we may bring in to help analyse our web traffic and suggest improvements to our site.

We will only disclose your personal information to internal employees and contractors of Podcura foot health Ltd who administer any accounts, products and services provided to you by us, as well as third parties for credit card processing and delivery of your orders.

To protect both you and us from fraud and theft, we may disclose your details to other organisations (including law enforcement agencies) involved in fraud prevention and detection.

We may disclose your personal information to anyone in the context of a sale or merger of all or part of our business, to anyone to whom we transfer our rights and duties under our agreement with you, or when we must do so by law.

We only accept card payment using Square to keep your details as secure as possible at all times, Square’s privacy and security policy may be viewed here

 

We also maintain physical and procedural safeguards in connection with the collection, storage, and disclosure of personal information.  This includes ensuring that only employees who need the information to perform a specific job (like billing clerk or customer service representative) are granted access to personally identifiable information.  All employees are trained and regularly updated on security and privacy practices.

 

Any personal information you give us will be treated with the care and respect that it deserves and in a manner of your choosing, and will always be held securely. We only ever collect the data we need and you will only ever receive marketing emails from us if you choose to do so. Your data will never be sold or passed on to anyone other than internal employees and contractors of Podcura foot health Ltd.

The Information we collect during an appointment

What information is being collected?

Any relevant personal information collected at the initial appointment time by us is a legal requirement. This information is collected and recorded hard copy on a patient record card. This process is one to one with the patient and Kelly Ashforth only. Personal information we collect includes name, address, email address, date of birth, diabetic status, any allergies, medications, current infections, and the name and address of your GP. During your first visit you will be asked to sign a GDPR consent form which outline the information we collect and what it is used for.

 

How will this information be used?

This record card is used to collect relevant medical information before any clinical treatment. We need to use this information to diagnose any foot health conditions and ensure any treatment we give you is suitable based on medical conditions or medication taken for your safety.

 

How long do we keep this data?

Record cards are kept for a minimum of 7 years following your last treatment or until the age of 25 if you are under the age of 18 at your last treatment date.  Your details will be destroyed after this period.

How is this information secured?

Patient records are stored securely in a locked cabinet, only accessible by Kelly Ashforth. Patient records must be updated where necessary and treatment notes recorded at each treatment appointment. This is a legal requirement. As such we only keep hard copies of our patient records.

 

Whom will this information be shared with?

Patient record card information is unshared and used solely by Kelly Ashforth before any treatment.

Subscription to our marketing emails

On the podcura website, users are given the opportunity to subscribe to our marketing emails. The signup form used for this purpose asks for your name and email address.
